First Visit

On your first visit you will be greeted by our front office staff and be welcomed as the newest member of our family. You will then be asked to fill out our required forms or you may request to have the forms emailed to you so you can fill them out in advance. Dr. Kampourakis will then discuss with you your overall health and your past dental history. She will then explain his theory of care and spend some time discussing prevention of dental disease. Each new patient is treated as an individual with their own professional dental needs. Also on your first visit any necessary x-rays will be taken. a thorough oral exam will be done then Dr. Kampourakis will discuss with you any findings. No treatment will be done without your consent.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit. Please alert the office if you have a medical condition that may be of concern prior to your treatment (i.e. diabetes. high blood pressure. artificial heart valves and joints. rheumatic fever. etc.) or if you are on any medication (i.e. heart medications. aspirin. anticoagulant therapy. etc.) or require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), or a FMX (within 5-years) you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
